Instructions:
1. Collect the items to be made into a memory blanket. To determine the number of items you
will need, see below. If both sides of a piece of clothing can be used, count that item as two.
2. Choose your blanket size.
32"X32" = $75 (16 pieces of children's clothing)
40"x40" = $110 (25 pieces of children's clothing)
48"x48" = $160 (36 pieces of children's clothing)
64"x72" = $320 (72 pieces of children's clothing)
3. Choose the fabric for the back of the blanket.
4. Enclose the following in your package:
-Items to be made into a memory blanket
-Memory Blanket Order Form
-Check or Money Order
5. Mail your package to:
Willow Creek Baby
6207 Crofton Court
Mechanicsburg, PA 17050
6. Your custom-made Memory Blanket will be shipped to you within 4-6 weeks upon our receipt
of the items listed above.
