Instructions:
1.  Collect the items to be made into a memory blanket.  To determine the number of items you
will need, see below.  If both sides of a piece of clothing can be used, count that item as two.

2.   Choose your blanket size.
32" X 32" = $80 (16 pieces of children's clothing)
40" x 40" = $125 (25 pieces of children's clothing)
48" x 48" = $180 (36 pieces of children's clothing)
64" x 72" (twin) = $360 (72 pieces of children's clothing)
88" x 96" (queen) = $660 (132 pieces of children's clothing)

3.  Choose the fabric for the back of the blanket.

4.  Enclose the following in your package:
-Items to be made into a memory blanket
-Memory Blanket Order Form
-Check/Money Order or Credit Card (Master Card and Visa)

(Please note that we do not return any scraps or unused clothing unless you request
that we do so on your order form.)

5.  Mail your package to:

6.  Your custom-made Memory Blanket will be shipped to you within 5-6 weeks upon our receipt
of the items listed above.
