Instructions:
1. Collect the items to be made into a memory blanket. To determine the number of items you will
need, see below. If both sides of a piece of clothing can be used, count that item as two.
2. Choose your blanket size.
48" x 48" = $145 (16 pieces of teen/adult's clothing)
60" x 72" (twin) = $270 (30 pieces of teen/adult's clothing)
84" x 96" (queen) = $500 (56 pieces of teen/adult's clothing)
3. Pack up your clothing items, order form and payment, (MC, Visa or check) and mail it to the
address below.
4. Mail your package to:

Christmas orders must be received before November 1, 2011 to guarantee a
Christmas delivery. Or select "rush order" for a 3-4 week delivery time.
